Order matters more than the list: the microchip goes in before the rabies vaccination, or the vaccination does not count.

An ISO-compatible microchip is required for dogs from high-risk countries and recommended for all imports into the US.
Dogs that have been in a high-risk country for dog rabies within the past 6 months are required to be vaccinated against rabies in order to enter the U.S.
Dogs must have a CDC Dog Import Form receipt; APHIS recommends a veterinary health certificate. Cats must be healthy on arrival and require no rabies documentation.
