These are the country's rules. Airlines refuse some breeds on their own — their limits are in the carrier table above.
The simplified procedure applies while there are no more than TWO animals per person. A third animal turns the trip into an ordinary import: a permit from the Chief State Veterinary Inspector is required, preceded by an application to the regional veterinary directorate, a review of the epizootic situation within a week (up to three weeks if quarantine is needed) and three more days for the decision itself. That is a month or more of correspondence, not a formality.
If a dangerous animal disease breaks out in the country of departure, the Chief State Veterinary Inspector may suspend an already issued permit and refuse entry — for up to one month, while tests are carried out. This does not directly affect a personal pet under the simplified procedure, but it shows that the decision at the border always rests with the veterinary service.

On import, the veterinary certificate is issued by the veterinary service of the country of DEPARTURE — not by the Uzbek side. At the border the veterinary control post then issues a veterinary attestation. For a personal pet (no more than two head) both documents are issued WITHOUT a permit from the Chief State Veterinary Inspector — that is the simplified procedure.
The order matters more than the list: a step done too early has to be repeated. The intervals come from the same rule fields the engine uses for the earliest departure date.

How to take a dog or cat out of Uzbekistan when the owner travels with the pet (non-commercial). Steps run in order; the depth of chip and lab requirements depends on the destination country, while the paperwork is issued by Uzbekistan's state veterinary service.
For most far-abroad destinations (EU, USA, UK, etc.) the pet is identified with a 15-digit ISO 11784/11785 microchip. Uzbek law does not itself require a chip to leave — this is a destination-country requirement, but without it an international certificate for travel beyond the CIS is of little use. For the EU and several countries the chip must be implanted NO LATER than the first rabies vaccine: if vaccinated before chipping, the vaccination must be repeated. For CIS destinations a chip is usually not required.
Required. Animals over 3 months are vaccinated against rabies, and the shot must be valid — typically given no more than 12 months before travel and at least 30 days before departure (for far-abroad destinations this immunity-build period is observed). All shots are recorded in the animal's vet passport. The exact minimum interval and validity are set by the destination country, so check its entry rules.
A rabies antibody blood test is needed not for leaving Uzbekistan itself, but to enter countries that require it (EU, Japan, USA, UAE, Israel, Korea, Singapore, etc.). The level must be ≥ 0.5 IU/ml, blood drawn no sooner than 30 days after the vaccine. For EU entry a 3-month wait after the blood draw usually applies. Not required for CIS/EAEU. Note: Uzbekistan may not have an EU-accredited lab — check in advance where to send the sample, as this is the longest-lead step.
Before travel the pet is examined by the state veterinary service at the owner's place of residence or where the animal is kept. There the animal is examined, vaccinations are checked against the vet passport, and a veterinary certificate of health is issued. This is Uzbekistan's domestic document: it confirms the animal's health and serves as the basis for the international export certificate issued at the border. Private clinics do not issue this state document — the state vet service is required.
On departure day (or shortly before) at the border veterinary control point in Tashkent airport a state vet examines the animal, checks the chip, vaccinations and the domestic certificate, and issues the international certificate. For travel beyond the CIS a Form 5 veterinary certificate (5a–5f) is issued; for CIS countries a Form 1/2/3 certificate. Documents are issued in Russian, Uzbek or English; keep the vet passport and the titer report (if your destination needs it) with you. Duplicate certificates are issued on the exporter's request.
A relief applies to personal pets: up to 2 dogs/cats (and up to 10 birds) per passenger may be taken out WITHOUT a separate permit from Uzbekistan's Chief State Veterinary Inspector. For more animals his permit is required — a decision is made within three days, so plan such cases ahead. For a typical trip with one pet no permit is needed.
