Indonesia sets its own requirements for bringing in dogs and cats. From any departure country your pet needs import permit, rabies vaccination, additional vaccination, parasite treatment, veterinary health certificate, rabies antibody titre test and pet passport. The rules are the same for dogs and cats.
Quarantine on arrival is mandatory — 14 days; documents do not waive it. Pets may not enter through just any airport: there are 2 designated points. A pet may be brought into Indonesia only through designated points of entry with quarantine offices: Soekarno-Hatta (Jakarta, CGK) and Surabaya (SUB). Bali (Denpasar) is not among them — but not because it is «rabies-free»: Bali is in fact listed among the rabies-infected territories of Indonesia, and only ten provinces out of thirty-three are free of it.

Mandatory import permit (Surat Izin Pemasukan) from the Directorate of Livestock Services & Animal Health, Ministry of Agriculture. Obtain in advance, 1-2 months lead time. Also: notify the quarantine officer at least 2 days before arrival; the pet must not be pregnant or lactating.
A pet passport is accepted INSTEAD of the veterinary certificate. This cancels no procedure: the microchip, the rabies vaccination and — where required — the antibody titre remain mandatory. The passport merely attests that they were done, and takes the place of the document otherwise issued for the trip. That is exactly why it is more convenient: a certificate lasts days, a passport lasts years, and it shows the vaccination never lapsed.

The order matters more than the list: a step done too early has to be repeated. The intervals come from the same rule fields the engine uses for the earliest departure date.
